Steps
1. Click on 'Profile'
2. Click 'Help' at the top left corner of your digital cert
3. Select '7 I would like to report a problem other than the one above related to my digital vaccination certificate'
4. Select 'My name in the digital certificate is wrong'
5. Submit your answer
